Document Commissioning

Under the Commissioners for Taking Affidavits Act, Municipal Clerks and designated officials of the Township of Blandford-Blenheim (also known as Commissioners of Oaths) are authorized to take certain types of declarations and to administer oaths.

Please note that the Township of Blandford-Blenheim does not have a Notary Public on staff.

Affidavits

Affidavits are verified documents containing sworn statements. The Township can commission affidavits provided that:

  • All individuals required to sign are present.

  • Valid government-issued photo identification is shown.

  • Documents must be signed in front of the Commissioner of Oaths, not in advance.

Fees

  • $10.00 (includes HST)

Commissioning Process

The Clerk or their designate (Commissioner of Oaths) will review your document to confirm it meets Township requirements before it can be commissioned.

Before You Visit

Please contact the Township Office to:

  • Confirm that a Commissioner of Oaths is available.

  • Determine whether your document can be commissioned.

  • Confirm the applicable fee.

The Township of Blandford-Blenheim reserves the right to refuse commissioning services for any document.

  • Statutory Declaration for OSAP – Statement of Common Law Status/Supporting Children
  • Affidavit of Unregistered Vehicle
  • Immunization Health Form
  • Statutory Declaration of Common-law Union (single signature) - Service Canada form
  • Application for Change of Sex Designation on a Birth Registration

  • Wills, living wills, estate documents
  • Power of attorney documents
  • Divorce, separation, or custody documents
  • Cohabitation agreements
  • Real estate or land transfer forms
  • Court or civil litigation documents
  • Letters of invitation for immigration/visa purposes
  • Travel affidavits for minors
